This product ships to you and ready to use! There is no-need to search through dozens of digital forms and figure out how to sort it all out and organize it. I have already done that for you! This binder mirrors exactly what I use for Ava and I take it to all the IEP Meetings, Doctor/Specialty Appointments, Parent-Teacher conferences, and when discussing goals for Ava with the OT and Speech therapists.
Our Care binder is well-organized, well-thought out and color-coded to keep everything you need for your child in ONE place! 1. Planning- includes a blank monthly overview calendar, 12 printed copies plus pdf download
3 yellow cardstock dividers.
In this section I keep the school year calendar, classroom schedule, and appointments
2. Tracking- includes a meltdown tracking form, 5 printed copies plus pdf download. A meals tracking sheet, 8 printed copies plus pdf download, and 3 orange cardstock dividers.
In this section I keep tracking sheets provided by teachers and therapists
3. Medical- includes a medical emergency form 2 pages, 1 printed copy plus pdf download, medical/specialty appointment information sheet, 5 printed copies plus pdf download, and a medicine/illness tracking sheet, 3 printed copies plus pdf download. 3 red cardstock dividers.
In this section I keep a copy of medical insurance card, immunizations, referrals, doctor notes, and a detailed medical history report with dates and procedures.
4. Diagnosis and Evaluations- includes 3 pink cardstock dividers.
**There are no printed forms or pdf downloads for this section.
In this section I keep Ava's diagnosis form signed by her doctor as well as evaluation reports completed by the school, OT, speech, etc.
5. IEP Meetings- includes an IEP Meeting tracker (I use this to form to give a short summary of each meeting. It makes referring back easy to find and remember) 1 printed copy plus pdf download
IEP Meeting Questions and concerns form (I use this to write down my concerns throughout the school year, again so I don't forget, and I bring it to the next meeting) 5 printed copies plus pdf download. 3 purple cardstock dividers.
In this section, keep the copy of IEP Meeting here.
And yes, I replace the last copy of IEP Meeting with the newest one and I file the old ones in the cabinet, dated, so that I have them. If I need to refer back to one in the cabinet, I use the IEP Meeting tracking sheet to find the summary and date of meeting I'm looking for.
Everything is completely organized and well thought out!
6. Report cards and Progress reports- includes 3 dark blue cardstock dividers.
**There are no printed forms or pdf downloads for this section.
In this section I keep a copy of Ava's latest report card and progress report from school, progress reports from her therapies, school test scores, and parent teacher conferences. I also keep a couple of worksheets and writing assignments from school to track her academic progression, I always discuss these with her doctors and therapists.
7. Goals- includes a goals action plan form, 3 printed copies plus pdf download. 3 light blue cardstock dividers.
In this section I keep updated goals from therapies and school.
8. Resources- includes a sheet for notes, 5 printed copies plus pdf download. (My notes sheet contains large blocks throughout the sheet of lines. These are my "summary" blocks, so it makes it easier to find a specific note) I take notes on research, from doctors and nurses, therapists, and helpful tools.

In this section I also keep flyers, list of state/government phone numbers that might be helpful to Ava, and lists of doctors, specialists, and therapies.
